By law we are required to:
- assure medical information that identifies you is kept private,
- give you a copy of our Privacy Practices and legal duties with respect to medical information about you, and
- follow the terms of the Privacy Notice that is currently in effect.
Nova Behavioral Health, Inc. will only disclose medical information with your written permission to do so. If you provide us permission to use or disclose medical information about you, you may revoke that permission, in writing, at any time. If you revoke your permission, we will no longer use or disclose medical information about you for the reasons covered by your written authorization.
If you believe your rights have been violated, you may file a complaint with the agency or with the Secretary of the Department of Health and Human Services.
- All complaints must be in writing.
- To file a complaint with the agency, contact the Privacy Officer.
- We will investigate all complaints and you will not be penalized or discriminated against for filing a complaint.
